Loreen is kind, patient and caring. She assisted one of her patients on the 7 West unit by helping him get his dialysis setup upon discharge. As this was his first time receiving dialysis, he was overwhelmed with all the new information and appointments that needed to be arranged. Loreen worked with the patient's care team to get everything ready before he left the hospital to ease his transition and help ensure a seamless continuum of care.
