Lori collaborates with the other members of HBPC interdisciplinary team to ensure that veterans' needs are being met.
Lori displays the ICARE principles in her daily work in the HBPC program. She does not hesitate to cover for another nurse in their absence, ensuring that the needs of the veteran are made a priority. She will contact other disciplines at the VA or providers in the community to communicate the needs of her patients to ensure they are receiving the care they deserve. She can be found collaborating appointments with neurology for veterans that reside in ALF in Fairfield making adjustments to her schedule to accommodate the specialty clinics.

Lori is a resource for new nurses in the HBPC program and makes herself available via telephone, team, or outlook to assist them in providing care for this vulnerable population. She has taken on extra duties such as collecting infection control data regarding the use of catheters and UTI occurrences for the HBPC program. Her work is thorough and complete, care plans are continually updated and made available for other team members to refer to. Lori collaborates with the other members of HBPC interdisciplinary team to ensure that veterans' needs are being met.

Recently she worked with therapy regarding adjustments to equipment that could result in potential for falls and with SW to ensure hospitalized veterans would be discharged to the proper level of care for rehabilitation. Lori covered my panel of patients while I was out on an extended medical leave, and they were very well taken care of. She even stepped in and assisted a veteran shower in his home when the caregivers did not show up for several days. She did this because the veteran was in need, not because it was her job.
