Luz has done an exceptional job with a particular cardiac patient and her family who have been challenging throughout our episode of care. This patient has requested Luz to be her case manager when no one else fit the bill. Luz has gone above and beyond to meet this patients satisfaction needs. She rearranged her schedule to meet this patient's needs. Luz also is willing to go out of her area to deliver supplies to patients on weekends. It is a perfect example of team player.
