A patient was admitted to the rehab unit after suffering a stroke that left him needing complex care. The patient's family wish was to take him home. Before they could leave extensive nursing education was needed to take care of their loved one at home. Lynda was often assigned to care for the veteran and quickly gained the trust of the veteran and family. The patient and family developed a special connection with Lynda. Her thorough assessment and holistic care of patient and family gains not only their trust but that of the interdisciplinary team.
Lynda presents herself to veterans with a unique human- to- human interaction and touch which gains the respect of her patients. Lynda's communication with patients is respectful of their needs and desires for treatment plan but encourages to fully participate in the difficult rehabilitation process. Lynda's nursing care is highly regarded by our rehabilitation physicians.
Lynda advances nursing practice by keeping her certifications, attending conferences, and leading by example. She is an avid advocate to ensure patient's progress and concerns are addressed. Lynda's commitment to excellence; she takes on challenges and is not satisfied until she sees the fruit of her hard work. A family recognized Lynda as an exemplary nurse.
Lynda remained grounded with a caring approach through difficult times when caring for the patient. Although progress was evident but slow with often medical setbacks occurring. Lynda continued to create a healing environment to assist and support patient and family with physical and emotional needs.
Lynda was committed to providing the patient with quality care but what the family most appreciated was the comprehensive discharge education to safely care for the veteran. Lynda planned ahead often communicating with interdisciplinary team members to ensure that the patient and family would have everything prior to discharge. The family was grateful verbalizing, "Lynda is a caring and patient we are thankful for all the comprehensive teaching such as crushing medications, teaching tube feedings and providing stroke education as well.".
