One of Mae’s strong points is that she helps facilitate and direct the discussion of the patient’s care coordination and transitions of care during the daily interdisciplinary team (IDT) rounds.
Mae Thammavongxay, RN has 20 years of experience as a professional registered nurse (RN). Mae has been working here in JJP VA Medical Center for 18 years and has been in her current role as the Nurse Navigator for 6 years. She had made major milestones in her professional career and makes a difference in various positions she held within the organization from being a staff nurse and charge nurse in the ICU and outpatient areas, a clinical course and continuing education coordinator, video telehealth, and nurse navigator.

In her current role, Mae demonstrated effective and quality nursing practice in positive ways. The Oryx measures are steadily improving and have been above the VISN2 and national average for all quarters of the FY 2022. Mae totally understands the important role that she plays throughout the patients’ length of stay.

Establishes and maintains an educational plan to support professional career development with demonstrated innovative outcomes in nursing practice. Mae has provided the education to different JJP team members at different levels in all the positions she held. In her role as the Course and Continuing Education coordinator, Mae has developed a teaching plan and provides education to nurses and providers regarding the Global Metric 90 for the hospital. Mae has created a video training for providers to correctly clear the substance 40 and tobacco 40 reminder for Global Metric 90 for the JJP VAMC. Mae has coordinated with the clinical care team manager, staff nurse for the End-of-Life Nursing Education Consortium training class for the Critical Care and Medical-Surgical staff at the JJP-VA Medical Center in September and December of 2021 wherein she was also one of the presenters.

Her strength in interdisciplinary collaboration and positive approach to facilitation of the daily rounds show during this IDT patient rounds.
