Maile Fitzgerald, RN
Maile has shown exemplary skills working with our patients and their families to coordinate a discharge plan. in one instance, one of her patients could not return to their long-term home in a facility due to a complicated disease process and new tracheotomy. The patient had been at her previous long-term facility for seven years. Maile showed extreme compassion for the patient's mother and sister during this difficult time.
As a case manager, she had reached out to several facilities out of state so the patient would be closer to her mother, as well as local facilities that would be able to handle a new unvented tracheotomy. Through the process, Maile was effectively communicative with the family. She made them feel part of the decision process as well, comforting them during this lifestyle change and transition to a new discharge plan. Maile successfully placed the patient in a new facility in Manhattan in a timely manner with the mother's consent. The mother expressed her appreciation of Maile's kindness and professionalism.
Maile exemplifies the organization's core values and is excellent in all roles within her practice as a case manager. She is a team player and a resource for all clinical staff. She is a joy to work with as she makes our office a happier place with her presence.
