Marcia Davis received a report from an outside facility for a patient admission to the Hospice Unit. The veteran’s demographics matched the description of the patient with the report obtained, upon careful review of the information received, nurse Davis discovered the veteran’s date of birth and social security number were incorrect. Due to the medical condition, the veteran was unable to communicate and confirm his own identity. Marcia's attention to detail set the standard as a true advocate for patient safety; she immediately called back the sending facility to corroborate the patient’s personal information; established contact with the veteran’s emergency contact and informed the Administrative Officer of the Day (AOD) of the inaccuracies of information sent by the discharging facility.
