The patient was in the hospital for 304 days before discharging home with many complex medical needs, including home dialysis, home high-level respiratory support, and numerous home equipment needs. Marisa even located a set of SCD's in Sioux Falls for home use. Aside from the complexity of the patient's home medical needs, Marisa worked with the family to address their concerns, speaking to them on a regular basis with utmost attentiveness.
Marisa coordinated for the support services to be available to meet the patient and wife at home on the day of discharge to ensure a smooth transition home. She even arranged for enough continuity of care, setting up a PCP who was able to meet with this very complex patient and ensuring the nephrology team was able to safely transition this patient to home cares.
Marisa certainly demonstrates and embodies the "clinical expertise and compassionate, extraordinary care" of the DAISY Award. Beyond this, Marisa persevered through a very challenging social, medical, and ethical situation, and determination in fulfilling the primary wish of the family to bring their loved one home.
