Martha is very empathetic and cares so much about her members and others. She continually listens to their needs and tries to make their lives better.
Martha from Community First Health Plan is such a dedicated employee. She takes on so much and still is able to assist staff whether it be SUPERB training or answering questions from staff. There are so many great things about Martha. She is a wonderful human being and shows it through caring for her members and her peers. She goes above and beyond while being personal and professional with such vulnerable members. She has a unique ability to connect with her members. She is very empathetic and cares so much about her members and others. She continually listens to their needs and tries to make their lives better. She is always on the go and wants to resolve patient issues. She does not stop until the task at hand gets done. There is not one specific member but multiple members she works with that she has impacted throughout her almost 5-year career at CFHP. As her manager, I have had the honor of speaking with some of her members who have explained that she is a "Godsend." One parent explained that she thought she was unable to care for her child until Martha came along.
The most recent member that Martha helped was evicted from their home after the "freeze". Martha quickly jumped at the opportunity to help this member. She worked with the nursing agency to come up with a plan to get the member and family back in their home after they were evicted. She educated the family on the moratorium and contacted the landlord advising them of the moratorium which resulted in the family going back into the home. Martha did not stop there, she has worked countless hours trying to come up with a more permanent resolution for this family as her member was very fragile, on a ventilator, and receiving private duty nursing services. She knew that the family would be evicted after the moratorium ended and did not hesitate to continue working with different agencies to include the state representative office. She wrote in requesting them to assist with housing. She worked with multiple agencies until that call came in from the family stating they received housing through SAHA. Mom is very happy and excited that she now has a 3 bedroom apartment that she can afford and will not be homeless. Our team is super excited for this family, especially Martha! She is definitely "a force to be reckoned with". I am extremely proud of her and all that she does and continues to accomplish.
