I have been working with Ms. Chery for nearly three years. She consistently delivers expert care, leadership, and quality service. She does an excellent job of working both independently and collaboratively.

There are numerous examples of Ms. Chery's dedication and quality service. This first being a situation in which a patient arrived twenty-five minutes late to the clinic for his primary care appointment and was advised to re-schedule. The patient got anxious and refused to leave the clinic without being seen. The patient had mental health limitations and was beginning to escalate his behavior and verbal outbursts. She promptly took control of the situation and attended to the patient's health needs in a calm manner by quickly assessing the veteran's physical and mental health needs while considering patient and staff safety. With the help of Ms. Chery, the patient was able to gain his emotional composure. He was able to cooperate fully and comply with clinic rules and regulations. Upon further assessment of the patient, he verbalized chest discomfort. Ms. Chery collaborated with the primary care team and coordinated his care with the psychiatry staff. She reassured him that the team would help him. She assisted the patient to a private exam room, and directed a staff member to remain with the patient while gathering an EKG and vital signs. Simultaneously, Ms. Chery encouraged the PCP to see the patient and enter a stat EKG order. Patient was stabilized and agreed to further cardiac testing. Ms. Chery's intervention demonstrated ICARE values and adhered to the 3 pillars of a High Reliability Organization by: 1) Conscientious use of her expertise that promotes process improvement by diligently attending to patient's needs which also promotes 2) Patient and Safety Culture · and 3) A Radical Commitment to Teamwork. Ms. Chery's team spirit and selfless act is well-deserving of the Daisy Award with no reservations.

The second most recent case happened last week. One of my patients called the clinic in distress. The patient had just been discharged from the hospital the day before the call due to an altered mental status. The patient was with family in another state and was staying at a hotel during the call. Ms. Chery was alerted of the situation. Ms. Chery took over the call, and managed to calm down the patient while simultaneously communicating with Suicide Prevention Coordinator via Teams. Ms. Chery reassured the patient's safety, and gathered pertinent collateral information from the patient's guardian, and assisted the veteran with her acute psychiatric hospitalization. The patient arrived safely to the psychiatric ward and continues to receive treatment for her illness. Because of her collaboration with the interdisciplinary team and the family she was able to help the patient by providing psychological support, reassurance, quality care, and safety. This case is one of the many cases I have witnessed while working with Ms. Chery. This case is another example of her commitment to a high reliability organization standard to promote and support its pillar of 1) Conscientious Use of an Improvement Process Platform. 2) Patient Safety Culture and 3) A Radical Commitment to Teamwork.
