Matthew Berman, RN, BSN
Matt works as a Heart Failure Navigator Nurse, and often times with some of our sickest patients who have very limited financial resources. A patient came to him with food concerns and told him about some packaged food meals that a food bank was offering, which seemed to be low in sodium. With Matt's help, a project was spearheaded, where Matt worked with this patient, to provide a letter to the food bank from a provider (me) on the importance of low-sodium meals to heart failure patients. Matt was able to somehow get a designated freezer at the food bank, filled with low-sodium prepackaged meals, just for heart failure patients in need. By working with our patient who voiced his own personal need and a simple idea, Matt was able to help provide a resource for our entire community, which is absolutely amazing. It will help cut down on hospitalizations, exacerbations of heart failure in patients, and, simply put, hunger. I am so proud of Matt, his heart for patients, and his creativity and hard work. Thank you for being a true advocate for our most vulnerable populations, for working not only within the clinic and hospital to help, but within the community to represent what a nurse truly is. You have helped to make an impactful difference in lives you will never even be aware you have touched.
