Medical Home Port Team

Pam Brosius, Pam Hayes, and Paula Smith were recognized for their individual and team efforts in the care of one of our tiniest patients and his family. After needing urgent open heart surgery at only four days of life, this patient and his family had many needs taken care of by this great team of nurses. They assisted with everything from breastfeeding support, weight gain, referrals and coordination of care during a long hospital stay, complicated follow-up needs, and most importantly, compassion and support for baby, mom, and family during this difficult time.
