I am very excited to have the opportunity to nominate Melinda Conley for the DAISY Award. Throughout this past year, Melinda had produced phenomenal results and given selflessly with her time. Melinda has been a driving force in bringing our clinical results from the low 40's to consistently being in the top 20% of the company. She never settles or accepts a patient’s results are good enough. She holds herself to the highest standard and consistently pushes our patients and team to be better. She does all this while having an amazing relationship with our team and our patients. Both our clinic engagement and CHAPS score for the clinic was in the top 95% of the company.
In 2020, Roseville became a COHORT Facility. Melinda immediately took a leadership role in adjusting our schedule and educating our team. Melinda has been selfless with her time throughout the past year. She has consistently worked every Friday and Saturday, in addition to her M-TH shift, in order to admit PUI-S patients at a moment's notice. This past year, Melinda has worked approximately 90% of her weekends off and admitted 350+ patients as a part of the COHORT. Melinda has a true passion for both Dialysis and DaVita. I am proud to have Melinda on my team.
