She advocates tirelessly and is constantly identifying additional ways that we can better support our patients.
Melissa has done an incredible job bringing to life the discharge coordinator role on the PP unit that aims to keep patients safe and well as they transition home. She ensures that all of our patients have everything that they need to be set up for success-- from medications to follow-up care to education to blood pressure machines. To do this she advocates tirelessly and is constantly identifying additional ways that we can better support our patients. The work that she has done over the past year and a half has made a significant impact on hundreds of patients' lives. I am so proud to be her co-worker. Here are just a few of the things that patients have shared about her:
“Melissa was amazing and thought of everything! Definitely made the process of going home easy!”
“A common line I heard in my PEPS group was ‘No one cared about mom after birth: everything was about baby.’ This helps take that feeling away because Melissa was asking questions about my goals in going home and what I was going to do to maintain my health/wellness.”
“Amazing! Melissa picked up my meds from the pharmacy for me because there were no meds to beds and my husband was waiting in the car with our 4-year-old.”
“I feel confident going home because of Melissa. So grateful this (discharge education) was a part of the process.”
