On the heels of attending the high reliability trainings, this weekend I witnessed one of the most well organized attention to detail processes that involved a medication error by one of my colleagues. Molly Armstrong in the NICU without a doubt deserves the DAISY Award for implementing and accessing her high reliability training. She escalated her concerns with a questioning attitude, clarified where in the process that a medication was drawn up incorrectly, yet labeled and scanned appropriately for this patient. Because she was this patient's bedside nurse both Friday and Saturday, she noted that the color of her medications were different from one to the next.
