Tach was involved in the thoughtful discharge of a patient who was changed to palliative care at very short notice. The patient’s wish was to not die in the hospital and to get home and have a beer and cigarette. Tach facilitated his discharge within hours. He was discharged home on Saturday and passed away two days later on Monday. Tach went above and beyond to organize a complex discharge with input from the OT, CHNS, HITH, and palliative care services. Great gratitude was passed on by the patient's family.
