A patient from out of state had some extenuating circumstances that caused her miss IV antibiotic treatments and did not have a current prescribing doctor.Nicki called the doctor the patient had previously seen for Home Health Vancomycin. The office would not write the order without all of the patient's information from the Home Infusion Service. Also, the doctor was out of the office for a few days and the patient did not want to wait any longer for treatment. Nicki then, in turn walked to the Chief Medical Officer's office and explained the patient's dilemma and requested a Vancomycin order for Home Health. The CMO came to the patient's room and interviewed the patient and then consented to write the orders for Home Health. Nicki tracked down the all the necessary paperwork which took a while. She had to call different places to obtain the necessary documentation and faxed it to the Home Infusion Service. The liaison from the infusion service came to the hospital to interview the patient and set up the infusion in her home to start immediately. This whole process to completion took about 3 hours or more. The patient and husband were extremely appreciative of her help.
