Patient Flow Coordination Center PFCC is a hidden Gem of the VA healthcare system. The Atlanta VA PFCC is the flagship unit of the VA HCS. PFCC team and leadership have trained in VISN 7 and other VISNs (NC, SC, TX, MS) to present 24/7 Flow Center to their leadership and setting up Flow Center. Patient flow must always be aware of the state of the house, and the community, to effectively move patients to where they can receive the level of care needed in a timely manner.

The Patient Flow Center facilitates transfers to and from VA and Non-VA hospitals in state and out of state. Many of these veterans being transferred are usually experiencing psychiatric or medical issues. It is the Patient flow coordinator’s responsibility to ensure transfers are coordinated appropriately and in a timely manner by collaborating with the physicians to determine the appropriate service needed and not necessarily the service requested.
