A patient presented to the Emergency Room after being kicked out of a homeless shelter that our hospital helped place her in after her last hospital admission in our facility. She had nowhere to go and no family to fall back on. Patti Croy attempted five different types of placement options to find placement to avoid being readmitted again. Another placement problem was the need to support her loving pet cat to be allowed to come with the patient to the nursing home. Working together, Patti and her team found a facility that would support this patient and allow visiting rights for her pet cat. A pet carrier and pet food were purchased by Patti and her team to assist with the patient's nursing home admission. The nursing home expressed their approval of how the SBL team supported this homeless patient. The patient has been found happier with expressing laughter and excited for the first time because she has someone who cares for her and a place to live.
