Patricia deserves recognition! She is a nurse home visitor for Nurse-Family Partnership, an evidence-based home visitation model that focuses on promoting positive healthy mother and child outcomes. Her heart is as big as a galaxy and she truly cares for the wellbeing of her clients and their families.
On the first postpartum visit to one of her high-risk teen clients who had an underlying hypercoagulability condition, Patricia assessed the client and recognized she was having pulmonary issues. Patricia acted quickly, instructing her to go to the ER and coordinated all logistics that go with that to make sure her sister and baby were safe. Patricia called and informed the client's mother who was at work, discussed having her younger sister walk to aunt's place of work to get the key to go to aunt's house and stay there as she had done this in the past; and having older brother drive client and her baby to the ER. Patricia followed and met the client at the ER for support and help with translation. The client was admitted and diagnosed with multiple blood clots in her lungs. Patricia stayed with her and her brother who was caring for the client's baby until the client's mother arrived. Patricia's nursing assessment skills and her quick action and coordination is a true testament of being an excellent nurse and she goes beyond expectation for her clients.
