Rachael is the most amazing case manager! She has the kindest way with patients. I have seen her de-escalate many situations by being calm and compassionate. Not only is she well-versed in her patients' history and needs, but she makes meaningful connections with their families as well. My office is next to Rachael’s, and I hear her on the phone speaking kindly and politely, calmly and compassionately to many patient family members. She maintains her poise and calm even in the toughest situations.

Rachael is an amazing partner! She always follows up on what she says she will follow up on. She rounds with the nurses and ensures patients are ready for a smooth transition of care. Rachael has handled some really difficult cases and does so without ever losing her cool. She is also innovative. We used to have a patient come in repeatedly for the same thing, and after some IV fluids and pain medications, the patient always went home. Rachael got creative and figured out how to get this patient IV fluids at home through a health service so that she didn't need to be readmitted to the hospital repeatedly.

Racheal is an uplifting spirit. She always has a small joke, a prayer, a hug, or a funny meme to make your day. She is one of the most sincere, kind, and compassionate human beings I've ever had the pleasure to work with. She is a true asset to this organization, and she embodies our Core Values daily.
