Clearly, Rachel made a huge difference in this situation. She was an exemplar of Primary Palliative Care Nursing.
Last week Rachel called Palliative Care to talk about her patient with very advanced cancer. He had young children and we asked about referring them for Child Life Services, and she had already reached out to them. We asked about his symptoms, and Rachel had already reached out to the primary team to ensure his symptoms were well managed. We asked about Spiritual support and she had already reached out to the Chaplain who had been checking on them and offering prayers. She reached out to the Palliative Care Service, and the family transitioned the patient to comfort care and could stay with him at this most important time. Clearly, Rachel made a huge difference in this situation. She was an exemplar of Primary Palliative Care Nursing. She was an exemplar of how amazing nurses at UCSF make a difference every day. I am so proud of her!!
