Rachel R Weaver, RN, BSN

Rachel Weaver works as a care manager nurse in the PM&R clinic in Wyoming. She always goes above and beyond in patient care. She speaks directly to patients and collaborates with multiple providers in the specialty team. She is patient and excels at answering questions, directing patients to self-advocate, and with empathic listening. In one specific instance, a patient had been turned down by multiple CITC neurosurgeons for consultation. His diagnosis and pathophysiology were complex. He also was found to have a mild cognitive disorder, which complicated his ability to understand an already challenging course and plan. He had multiple providers involved in his care, both in the VA and non-VA, who were attempting to treat this issue. Nurse Weaver spent countless hours on the phone with him coordinating care between VA and non-VA as well as explaining what was documented in his medical record. She ensured his VA medical record was up to date so that all VA providers would be aware of what was going on with the patient. She worked tirelessly to ensure the patient's questions were answered and that he got the best care.
