We had a patient who was nearing the end of his life, and his family wanted him to go home with Hospice. Due to the patient's medical complexity, several things (including a great deal of teaching) had to be completed prior to a safe discharge. Of course, this was all happening shortly before a weekend, so if everything was not accomplished quickly, the family feared their loved one might die in the hospital. Hospice does not admit patients to their service over weekends.
Roman was professional, calm, caring, and he saw to it that everything was done skillfully, quickly, efficiently, and with great care. His follow-up with me (SJH-Social Work) was fantastic and thorough. The family was able to take their loved one home because of Roman. Thank you so much for all you did!
