The Care Management Unit has a dedicated Nursing team that strives every day to provide the best quality of service to our beneficiaries empowering them in self-management techniques to take control of their chronic conditions. An example of the great commitment and service in the Unit is Rosa Ramos-RN, Case Manager. Rosa is a health professional who has demonstrated a great commitment to providing and treating our affiliates with empathy and sensitivity. She has a special touch which she utilizes to engage the affiliate in their self-management and helps them navigate through the continuum of care, coordinating all the services needed to assure quality health outcomes. She takes the time to listen to the needs of each patient and utilizes all resources available in the coordination to make sure that the affiliates receive all necessary services to keep them in their environment, with their family, support system and in an optimal state of health.
