Rubi Acosta is exceptional at her job as swing bed coordinator. Rubi's work during the past week exemplifies the care and compassion she extends to patients and their families. Mr. J. P. and Mr. L. C. B. both needed nursing home placement. Their families asked Rubi if she could find them places to reside and help get them accepted as new residents. Typically, nurses help provide lists of possible long term care facilities for families to use for visiting, calling, completing paperwork, and selecting a final home for their loved one. Loss of autonomy and long term care placement is a trying time for the individuals who will be relocating there and those who love and support them. 

Rubi went out of her way to help the patients and their families find the placement and went above and beyond to get the paperwork completed alongside them for their loved ones to have a new and most likely final home. She eased their way through a huge and monumental transition from swing bed admission into a long term care facility also commonly referred to as a nursing home.
