Sabina Okeke has been a Charge Nurse on 5W for many years. She has seen countless changes and always rises to the occasion. Sabina is extremely invested in the well-being of her patients.
Some examples include:
Offering encouragement to patients who have been bed-bound and prioritizing their mobility and personal dignity. She has worked with many patients who have come to our unit and have not been out of bed for a shower or even to use the toilet. She makes sure they get the proper room and equipment, always supporting their dignity. She sets goals, and when they meet them, there is much celebration. One example is a young bariatric patient who was unable to get off her couch at home. Sabina worked with her and got her out of bed, offering grooming and hygiene. She encouraged her to use her laptop as she
was a remote worker. She praised her and complimented her appearance. She worked closely with the family to get DME and have the patient return home.
This unit has many patients who stay for a long time. Some are not very attractive to other facilities and can be off-putting to others. Sabina finds a way to embrace these patients and make them feel like they are at home. I have seen her go out of her way to offer special treats to make them feel accepted. I have heard them call her sister or mother and tell her they love her. Once a placement is located, she will often provide a plan of care so the placement will be successful and the patient will not return to the hospital. Several patients have visited the unit after discharge, and it is wonderful to see their success.
Sabina coordinated and led a bedside care conference for a long length of stay patient. She identified barriers to discharge and set up a plan arriving at a discharge date just 5 days in the future.
Sabina does not let patients languish in bed. She looks for ways to help with the discharge process. Sabina partners with the Case Management team. She takes the initiative and follows up. She offers a wide variety of community resources and gets to know the family’s and patients’ individual needs. Sabina has a strong knowledge base for community resources such as the MS closet, adult family homes, HCS assessments, and Skilled nursing facilities.
