Sara Hart, RN

It is with great honor that I nominate Sara for the DAISY Award. My family recently had an elderly member admitted to TCRH who was quite ill and unfortunately had a poor prognosis. But we knew his wishes and we needed to develop an immediate plan for care as he was no longer able to return to his home. Sara was there, she listened, she cared, and she advocated for this patient. She was kind, professional, and compassionate. She immediately started making calls and referrals and within hours, she had secured a bed for him at a local facility and arranged for a hospice nurse to meet us there to initiate his end-of-life care. A true miracle!

Thank you to Sara for her wisdom and guidance, her compassion, and her empathy. It is clear she is very passionate about her job, and she performs it so very well. We are forever grateful for her aid and support in our time of need.
