Sara was nominated for helping coordinate care among a patient's biological mother and father, foster parents, and all disciplines involved in the patient's care. Sara ensured all parties involved were comfortable with the care the patient would now need to receive. Once she realized how critical the patient was going to be, she coordinated with the clinical director that all resources available, including the primary nurse system, were in place for this family. Sara had critical conversations with the patient ensuring the patient understood it was ok to be ok with the foster parents. Thank you, Sara, for your compassion and love for this patient.
