Sarah Edgell joined PeaceHealth this year as a Care Management RN and is consistently energetic and passionate about providing the best care.
She has demonstrated the skills to clearly communicate with patients, their families, and all the members of the care team.
She has navigated through multiple changes in the complex discharge planning for a patient who had little resources but had much need, she ensured compliance with a plan that would work for the patient.
She problem-solved and communicated the multiple changes with great grace and resilience going above and beyond to support the successful discharge of this patient back to the community after a lengthy hospitalization.
Extraordinary effort and skill, as well as compassion, were demonstrated by Sarah throughout the course of the discharge planning that took place over several weeks.
