Sarah made an awesome catch on Sunday. While sitting at the nurse station, Sarah noticed the patient become tachycardic on the monitor and then slip into a junctional rhythm momentarily. Sarah made the provider aware, and a troponin was added to the blood work. We ended up diagnosing an NSTEMI. Given that an EKG had already been performed earlier in the encounter and was normal and no cardiac labs had been ordered, this patient could’ve been discharged home without realizing that a cardiac event happened since he was not coming in for a cardiac complaint and never experienced chest pain while in the ED. Instead, he got admitted for appropriate care which certainly improved his outcomes.
