Sarah Susie, RN

We would like to nominate Sarah Susie for the DAISY Award. Sarah, as an RN and a clinic care coordinator, embodies the very best qualities of a nurse. She is empathetic, compassionate, exceedingly patient, fosters strong therapeutic relationships, and is a steadfast patient advocate.

Recently, a patient with whom Sarah has built a strong relationship through care management called her early in the day, stating that she was experiencing extreme anxiety and was thinking about calling EMS due to this. Sarah spent over an hour with her on the phone, mostly just listening to her and validating her feelings. This helped the patient get to a place where she felt safe and calm. Sarah then called her several more times through the day to check in with her and had the patient come in for nurse visit and blood pressure check as a way to meet with her during the day, get a face-to-face assessment and as a way to offer continued support to the patient. Sarah walked along beside this patient the whole day while also juggling her busy schedule of Medicare annual wellness visits and care management case load.
