Shane, without any previous Case Management experience, has jumped into this role at a time where the workload exceeds current staffing and has diligently and without complaint managed her patient census with empathy, efficiency. She consistently completes her work for safe and appropriate discharge planning for some of the most challenging patients in our health system. Despite the fact this was done with personal sacrifice of her personal time she has remained positive and proactive and has been a pleasure to work with.
She is a valued member of this inpatient team and deserves recognition for her contribution to safe and timely discharges and decreased readmissions.
