I had extensive goals of care discussions with the patient, and despite his relatively young age, he was "tired" and worsened to the point where a second goals of care conversation ensued, resulting in comfort care measures. The patient did not have anyone at bedside, so Shannon made it a point to hold the phone to his ears so that the patient could say goodbye to his brothers. I marveled as to the compassion that Shannon showed in this patient's transition to death and ensured that this patient did not die alone or scared and gave him a transition that I would want for any of my family members.
