The Case Management team works tirelessly to solve problems and ensure our patients have a clear, safe plan for discharge. From the moment patients are admitted, they are engaged in every aspect of their care, collaborating closely with departments across the hospital to coordinate services effectively.
Their knowledge of available resources and ability to problem-solve are truly unmatched. As patients’ needs become more complex, the team rises to the challenge every single day. They are creative, they research alternatives, they act as detectives, and most importantly, they show deep compassion for the patients and families they serve.
It’s a demanding role—like many others in the hospital—but with this exceptional group of men and women, we are proud and honored to have them as part of the Shannon family. The Case Management Department, including our social workers, nurses, and techs, goes above and beyond to live out the Shannon values every day—and then some.

Marty worked tirelessly with Hollie and the 4S case management team to secure hospice for a patient with limited resources in a very rural area. Marty is an excellent team member and fierce advocate for all of our patients, but especially our palliative care and hospice patients. Thank you, Marty, for your hard work on this complex and sensitive case.

Hollie has worked tirelessly on securing end of life services for a patient who lives a very remote area, with no insurance coverage for hospice. She has devoted many days to giving the patient and family the best chance of spending the patient's last days in her hometown, surrounded by loved ones. Thank you, Hollie, for going above and beyond to advocate for comfort, support, and dignity for your patient.

Laci, words cannot express how much we appreciate your efforts to find permanent housing for a homeless patient. Your patience was unwavering, and you remained professional every single time he was admitted. The situation was complex, frustrating, and often seemed hopeless. But you never gave up! You were firm yet compassionate, and your diligence helped find our patient a home! KUDOS to you for being an awesome patient advocate! We greatly appreciate you!

Last week, I was in the ER helping with a case. One of the providers commented to me how thankful he was to have Ryan in the ER. He described her as a "GEM" and commented on how helpful she was in her role. Ryan has been with us for a little over a year, but has done a great job acclimating to her role. We are so thankful to have her in the Case Management Department!
