Several weeks ago we had a patient who was mentally delayed and who was down in her luck. She had a rather complicated discharge plan that included taking the local bus to another stop in Key Largo and then traveling on to another part of the country with the total trip being 40 hours. Sharon was worried so she waited for the patient to be discharged late in the evening and went out of her way to help her get to where she needed to be.
This is not the only time Sharon has gone out of her way to provide excellent care for our patients. She takes her time to ensure every discharge is complete and the patient has everything set up ahead of time so they have a smooth transition of care. During call backs she has meticulously re-educated patients on their discharge instructions. One time Sharon had the call back patient come back to the hospital with all their medications and helped the patient sort through and discard the expired medications and re-wrote the medication instructions.
Sharon displays all the qualities of a DAISY Nurse including compassionate care, advocating for the patient, commitment to excellence, providing comfort, evidence based care, and patient and family centered care.
