There has been an increase in the number of patients with heart failure discharged from the hospital to skilled nursing facilities. These patients are often frail with significant comorbidity burden, mobility and cognitive impairments and inadequate home support. Sherry utilizes opportunities to improve management of using EBP on a daily basis. Her team efforts are outstanding and should be recognized.
