Stacey Hobson, RN
Stacey was a huge help with a patient who self-reported to be unsafe in their home after discharge from the hospital and refusal of SNF placement. We were in the process of hotlining him due to safety concerns and she took it upon herself while covering for me to check on the patient frequently (every 1-2 hours) by phone until we could verify his safety.
He had dropped his cell phone and was not answering so she called the patient's hometown police dept so they could do a welfare check on him. Thank God she did because he dropped his phone (he is wheelchair bound), had no water or food, and couldn't make it to the bathroom. The police showed up and helped him get his phone and water, and took him to the bathroom.
She continued calling him the rest of the day to check on him until the kids got off work. She is always taking it upon herself to be a great team member and patient advocate. Her effort and willingness to help know no bounds.
