Stacy Wilson RN, Team Leader, is a wonderful member of the SJ-SC case management team. We were challenged with a very difficult and complex discharge plan for a patient. The social workers and case managers worked tirelessly to find placement for this patient, whose specific needs and demands presented many challenges and barriers. Stacy was tasked to be the primary spokesperson to the patient and her support system. The nature of these conversations were often very difficult. Stacy maintained a calm, quiet manner at all times. She kept her focus on the patient's specific needs and to ensure a safe discharge plan for the patient. She made sure the patient and her support system had the appropriate resources and contacts provided to them so they could also be their own best advocates. Stacy demonstrated the SSM Health values by always treating the patient with respect. She followed our value of compassion by truly understanding the patient's unique circumstances and worked with her team to find amicable resolutions. She demonstrated our value of community - the complexity of the patient's needs stretched across multiple departments and Stacy actively worked to make sure everyone worked toward the common goal of a safe discharge for the patient. The saying goes, "it takes a village," which is true and because so many people and departments worked tirelessly with this patient and her support system. However, Stacy truly stepped up to be "the voice." When people commended her on what a good job she was doing, she would just smile, lower her head and say, "I'm just doing my job."
