The Stroke Team is the epitome of outstanding teamwork and collaboration. They have made a tremendous impact on the lives of stroke survivors cared for by our organization, going above and beyond to provide exceptional care prior to entering our doors, throughout the treatment process, post-discharge, and continuing long-term through our community support group. Surviving a stroke is just the beginning of a long recovery process that can feel isolating, overwhelming, and confusing for both the patient and their loved ones. The Stroke Team understands this and believes in ensuring patients and their families receive the education and support they need through their journey.

The moment a patient presents to the Emergency Department with signs and symptoms of a stroke, high-quality patient-centered care is delivered. The Stroke Team immediately begins creating an individualized care plan for that patient. For example, a young father who had a previous stroke at another healthcare facility did not have a positive experience. He experienced a second stroke, for which he turned to Stanford Health Care Tri-Valley to receive above-and-beyond care. This patient continues to stay in contact with the Stroke Team to give updates on how he is doing and seek continued guidance on diet-related challenges. The patient continues to state, “I just felt heard and supported by the Stroke Team at Tri-Valley.”

The team consistently scores an average of over 90% in the Stroke Education Satisfaction scores, and they successfully achieve follow-up with patients post-discharge more than 93% of the time. Following discharge, a growing number of patients and families are electing to stay connected with the team. The support group is enabling stroke survivors to build a strong community and maintain their relationships with Stanford Stroke clinicians. The value of the group was recently reflected by the request to add a second group each month.

All of this demonstrates the exceptional and compassionate care our stroke patients receive under the guidance of this team.
