Because she took the time to coordinate with correctional staff, she identified that the patient would likely be released from custody before his follow-up visit.
Sterling consistently goes above and beyond to ensure that vulnerable populations are given the greatest level of care at Shannon Clinic Orthopedics. Recently, an incarcerated patient with a fracture was referred to the orthopedic surgeon and subsequently had a surgical procedure that left him non-weight bearing and in a wheelchair for recovery. Sterling noticed that the patient would likely be unable to follow up due to a lack of transportation and social support if he were released from jail. Sterling took it upon herself to coordinate the follow-up appointment with the correctional health nursing staff at the jail to ensure that the patient would still be incarcerated and able to attend the follow-up visit. Because she took the time to coordinate with correctional staff, she identified that the patient would likely be released from custody before his follow-up visit. Sterling ensured that the follow-up visit was scheduled earlier so the patient would be present for the follow-up and also coordinated for the patient to receive equipment so he could remain non-weight bearing after being released from jail. Because Sterling saw a need and went out of her way to meet that need, our patient will have transportation to receive his follow-up care with the orthopedic surgeon, will have durable medical equipment needed once released from custody, and will have improved clinical outcomes recovering from his surgery. This will likely result in fewer 30-day readmissions and fewer unnecessary ER or Urgent Care visits as well. The DAISY Award is a symbol of special nurses who show true compassion to our patients in carrying out their daily duties.
