Susan followed homecare's process of calling the patient to schedule a visit for an admission appointment. She did not reach the patient. She then called the emergency contact and was told they had not heard from the patient for several weeks. She then did a drive by the home and did not get an answer at the door after knocking and yelling out the patient's name. The front of the home was obstructed with debris and the home looked abandoned.
Susan then called the authorities for a wellness check. Deputies were able to get into the home and found the patient in a back room with the home in disarray. The patient's physician and emergency contact were informed of the patient's condition and will follow up. DCF was also called for patient safety issues and possible self-neglect. Susan showed BayCare values and humanity in caring for our patients in the community. She truly is an extraordinary team member.
