December 2024 Honoree
Susannah Black, BSN, RN, OCN
There was a patient waiting for insurance authorization to get moved to hospice. The family was rightfully upset that it was going to take much longer than anticipated to wait on insurance. The family let Susannah know that this patient just wanted to be in Fremont, closer to family, to have one last family Christmas together with a tree and a turkey. Susannah got others notified to see if this process could be expedited as it was not communicated well that this patient only had days to live, and she did not want to die here without her family and last Christmas. Because Susannah was such an amazing advocate for this patient, the patient was transferred to hospice in Fremont that same day.
Thank you, Susannah, for your exceptional compassion, advocacy, and dedication to your patients. You truly make a difference.
