A teenage patient with HIV decided to stop taking his medication and stop coming to the clinic. Tami managed this situation with incredible insight and thoughtfulness. She managed to keep in touch with the patient, despite his open hostility and clear manipulation of ID team members. She recruited different team members with whom this patient had a history of rapport, to reach out to him and provide support.
Eventually, this patient returned to the clinic and got back on his meds with minimal harm to his overall health. Ultimately, this patient decided it was time to transition to an adult ID provider. After the referral was made and the patient was technically no longer under our care, Tami made sure he continued to get his medications and labs. When she saw that he wasn't yet scheduled for an appointment with Adult ID, she advocated for this patient and helped facilitate the transition of care.
