Tara was nominated by one of her colleagues. A patient was scheduled to be discharged to a long-term skilled nursing facility on a Sunday. There were many complications along the way, causing multiple postponements of the discharge/ transfer. She assisted in getting the patient transferred and was able to do it in a respectful manner (after numerous back-and-forth calls) so we still have a good working relationship with the facility. Tara always advocates for patient discharges, helps her coworkers, and is positive with a smile on her face. Tara advocates for each of her patients' individual needs, provides excellent care to each of her patients, and always helps her colleagues.
