Teresa has been working with a very difficult home care case for several months now. The particular patient that we share in primary care and home care has a poor living situation, decreased mobility, and limited resources with no support in the home. Teresa is very compassionate towards our patient and she has taken it upon herself to assist in getting supplies the patient needs. The patient was sitting in a wheelchair all the time and suffering from decubitus ulcers. Finally, we were able to get her a hospital bed, but then the bed had no linens. She took it upon herself to seek donations to help the patient have clean linens. Also, the patient only had a limited amount of clothing to choose from and she was able to get donations for new clothing as well as supplies to help keep the patient as clean and as dry as possible. Her dedication to this patient is admirable and it takes a special person to dedicate to these difficult and sometimes impossible situations. She has tried to advocate for the patient on numerous occasions to see about getting placement in other types of facilities where she will get the assistance she needs but the patient is unwilling because she does not want to leave her family as they "need her". Teresa is trying everything that she can for this particular patient and I do not want it to go unrecognized. I truly appreciate her efforts in collaborating care with our office to try and get this patient the best possible care we can provide and support her in whatever way we can.
