Teresa is a nurse case manager in St. Charles Prineville hospital. I meet with the team daily for case management so I see first hand how Teresa handles all the challenges of meeting her patients' physical, emotional and social needs. One of our patients lives alone in a low-income house apartment building. He has difficulties with self-care, medical needs, and social interactions. He came to our hospital with clothing that was soiled by urine and feces and most likely he had not bathed for months. After being treated for his medical needs that resulted from poor self-care and inadequate food choices, he was ready to be discharged home. Teresa announced to the team that this patient needed new clothes to return home and that she was going to take his clothing home on her lunch break and launder them. Teresa was able to encourage the patient to shower as we put his stiff, soiled clothing in a large garbage bag. He had an old coat that was soiled, and it would hardly fold in the bag. The patient then greeted all of us with a huge smile and new clothes that Teresa had arranged to be purchased and a bag of freshly laundered clothing to take home with him. This patient has never forgotten this kind gesture. When I work with him in our outpatient clinic and he sees Teresa he always waves and reminds me that Teresa is the nurse that cleaned his clothes and encouraged him to bathe. This patient has taken her kind gesture and encouraging words to heart as he now tries to change his clothes more often and shower at least monthly. Teresa is compassionate and a true advocate for all of her patients but especially the marginalized ones who have no one to look after them. She calls me frequently to follow a patient or do a home visit with someone who needs us to go the extra mile. Teresa is admired by all who work with her for her kind compassionate nursing.
