Had Terra not insisted his shoes be removed this patient would have succumbed to sepsis, she literally saved his life.
We received a patient via EMS for possible cold exposure. This patient has a history of schizophrenia and substance abuse and he is homeless. He arrived to our behavioral health unit covered in feces and urine. The decision was made to take him to the Hazmat shower. In the shower the patient was refusing to remove his boots, Terra recognizing the patient has diminished decision-making capabilities persisted with boot removal and ultimately cut the boots to remove them. The patient's feet were clearly affected by frostbite and the patient was moved to the main ED for a higher level of care. The patient was diagnosed with gangrene, and septic shock and went to the OR for bilateral below the knee amputations. Patients that suffer from mental health issues and/or substance abuse carry a stigma, they often refuse care and many healthcare providers give up on them. Had Terra not insisted his shoes be removed this patient would have succumbed to sepsis, she literally saved his life. A nurse who willingly does the "dirty work" is a nurse who deserves recognition.
