There was a particular patient on the unit who was admitted with a chronic medical condition; during her stay it was found that her living arrangements were less than optimal. As we discussed discharge planning the patient mentioned that her foster sisters were forcing her to perform inappropriate acts for money to continue to live with them. The only reason she was with the foster sisters at the time was because there was no one else that was willing to allow her to stay with them because they felt she was a financial burden, as her medical condition does not allow her to work. They would tell her "she utilizes too much water", or they were scared that she was a risk to an unborn child, as one of the people she stayed with briefly became pregnant and asked her to leave. Because of these reasons she ended up back with her foster sisters, feeling like she had nowhere to turn.

Once we heard the story on the unit, the case managers, social workers, and nurses immediately began to look into various resources and programs for this young patient. After options began to run thin and her discharge date came closer, the patient became more and more anxious about returning to the ill environment with her sisters. Little did we know that Terralyn had been in discussions with social services and the needed personnel to gather the appropriate permissions to take the patient home with her; as the details of the patients' story touched her heart. All the needed barriers were cleared and the patient is now living with and being cared for by Terralyn White.
