Theresa is an Extraordinary Nurse Care Manager. In the many years of my employment, she stands out as a top 10 nurse. Having been my employee for several years, Theresa consistently demonstrates the highest level of compassion and dedication to her patients and families. She sees all her patients daily and ensures their current and discharge planning needs are met to their complete satisfaction. She works at overcoming any obstacle which may interfere with patient choice. She ensures all patient needs are met for a safe and appropriate discharge. Her attention to detail is most admirable. Theresa consistently escalates patients with limited financial resources to rally support for charity funding. I have never received a patient service complaint from her patients/families since her mannerism and collaboration with them create a bond of trust. It is not unusual to learn she visited a patient discharged to SNF when she has concerns about the patient’s needs. Theresa even went to a patient's home to bring the food the patient needed. She is highly respected by her colleagues and our nursing and physician staff and is the first to ask if she can support those ill or in need. When one of our staff members had surgery, she went to her home daily to help with her needs. She is a true DAISY Nurse.
